1. FILLING IN A COMPLAINT FORM
A. NATURAL PERSONS
You can fill in the Complain Form:
- electronically in Word format, and then print it out and sign it, OR
- print it out in PDF form, complete it by hand and sign it
B. LEGAL PERSONS
You can fill in the Complain Form:

Remember:
- If you wish to have more than one person submit the same complaint, please have each person complete a separate Complaint Form.
- If the complaint involves two or more banks and/or investment firms, please complete separate Complaint Forms.
- If you are a company, please attach a photocopy of the Identity Card or valid Passport of the legal representative, his/her legalization documents, as well as the documentary evidence of the company’s last fiscal year’s turnover.
- If you have authorized a third person to represent you, it is necessary to attach a relevant AUTHORIZATION (under 2 below).
- Before sending the Complaints Form, please read carefully the “HFO PERSONAL DATA PROTECTION POLICY“. Given that for the consideration of your complaint and the legal processing of your personal data, your consent is necessary under the conditions stated in the above Policy, please, if you agree, indicate this by selecting the corresponding field “I CONSENT” in paragraph “DECLARATIONS / AUTHORIZATION” of the Complaint Form.
We inform you that if you select the field “I DO NOT CONSENT”, it is not possible to examine your case and all relevant correspondence and data will be destroyed/pseudonymized from our physical and electronic file, except for those deemed necessary to demonstrate compliance with our procedures, upon your notification.
2. FILLING IN AN AUTHORIZATION
You can fill in the Authorization Form and then print it and sign it, having your signature certified by a public authority.
