FILLING INSTRUCTIONS
· Please use a separate complaint form if more than one person is making the same complaint.
· Please use separate complaint forms if your complaint concerns two or more banks, investment firms or other financial services providers.
· If you are a company, please attach a photocopy of the identity card or valid passport of the legal representative, his/her legalization documents, as well as the documentary evidence of the company’s last fiscal year’s turnover.
· If you have authorized a third person to represent you, it is necessary to attach a relevant AUTHORIZATION with a notarized signature from a public authority. · Before submitting a complaint, please read the HFO PERSONAL DATA PROTECTION POLICY carefully. As your consent is necessary for the consideration of your complaint and the lawful processing of your personal data in accordance with the above policy, please tick the box in the “Declarations” section if you consent.
